Florida Hospital Flagler Cancer Institute understands that women with Breast or Gynecological cancer are faced with many challenges when they are given those cancer diagnoses.
WEB.jpg) Leslie Baker, our Cancer Nurse Navigator, is a registered nurse (RN) specializing in women’s health. She is here to assist with the cancer treatment plan your physicians have designed for you. She works closely with physicians and all other clinical staff to help you navigate through the continuum of care. She is a loyal source for support and resources and will be with you every step of the way in your cancer journey. That is why the Cancer Nurse Navigator is a key member of our multi-disciplinary health care team.
This service is available free of charge and is designed to help assist you through the process of diagnosis, treatment and recovery.
The Cancer Nurse Navigator provides the following services and support tailored to a patient's individual needs, including:
• Providing follow-up and ongoing support, before, during and after your treatment
• Providing materials and education about diagnosis, treatment and symptom management
• Coordination of diagnostic tests and treatments ordered by your physician
• Coordinating referrals to support services
Physicians, patients and family members can self-refer to the Cancer Nurse Navigator at any time. All calls are free and confidential, and no insurance is required to talk or meet with her.
