Florida Hospital Memorial Home Health is a not-for-profit Christian home health service owned by Adventist Health System and operated by Florida Hospital Flagler. The health care professionals at Florida Hospital Flagler understand that the home environment plays an important role in promoting health and recovery. Florida Hospital Memorial Home Health is a hospital-based home health care program that extends our ability to bring quality services into your home.
Florida Hospital Memorial Home Health is fully licensed by the Agency for Health Care Administration (license #HHA215580961), certified by Medicare, and accredited by the Joint Commission on Accreditation of Healthcare Organizations. Our services are available seven days a week and are provided without regard to age, sex, race, religion, handicap, national origin or sexual orientation.
What is Home Health?
Home care encompasses a wide range of health and social services that are delivered at home to recovering, disabled, chronically or terminally ill persons in need of medical, nursing, social or therapeutic treatment and/or assistance with the essential activities of daily living.

The home environment is more comfortable and conducive to resuming normal activities. You can sleep in your own bed, eat home-cooked meals and be surrounded by your family, friends and pets.

Home care may begin immediately after hospitalization or as a direct referral from your physician. A hospitalization is not required. All of us cherish our independence and the warmth of our own homes. Present technology allows many healthcare services to be offered at home. We are pleased to be able to offer these benefits to our patients.
